This site has been developed for patients with diabetes and for clinicians who provide diabetes care.  The MaineHealth Target Diabetes Program is a system-wide approach to diabetes care at several levels:
             - Patient
             - Community
             - Primary Care
             - Organizations

The overall goal of the Target Diabetes Program is to support patients in receiving the best care possible, and support clinicians in providing up-to-date quality services. 

The Target Diabetes Program is based on the Chronic Care Model and focuses on interventions aimed at improving diabetes by addressing the major components of chronic illness, which include:
            - Patient Education and Self Management
            - Healthcare Delivery Design
            - Provider Clinical Decision Support
            - Clinical Information Systems
